The transition to adulthood is a critical juncture for students with disabilities. Educators and other professionals can play a powerful role in equipping youth with the skills, experiences, supports, and linkages students need to have a future of flourishing. But what practices should we adopt as schools? What partnerships should we pursue? What might make a real difference in the trajectories of our students? This session will address research-based and recommended practices in the areas of transition assessment, transition planning, rigorous instruction, relevant experiences, fostering relationships, community linkages, and program evaluation. Attendees will leave with practical tools, resources, and ideas for supporting their students in effective ways.
